Chronic Disease management

Auxin’s Chronic Care Management  (CCM) program is a comprehensive, multidisciplinary intervention that is aimed to decrease exacerbations from chronic disease conditions. Our interactive platform allows us to  thoroughly design a care plan and also to create  productive follow up visits. We use an expert team of health care providers to deliver first in-class services to our patient population.  Our CCM program follow CMS guidelines and is designed to actively engage patients and educate them on their chronic conditions so they can adhere to lifestyle changes and medical interventions that will keep them away from hospitalizations and increase their healthy days.
