We divide our program in two:
1. Patients in the Hospital that can be discharge to our Hospital at Home Program.
2. Patients at home that can be diagnosed and treated to prevent ER visits and hospitalizations.

Every year millions of patients visit these facilities and are admitted to hospitals for acute conditions or chronic disease exacerbations that were potentially preventable if the patient had the right access to care at the right time.
At Auxin we are working together to reduce such potential admissions and increase healthy days while increasing value.
Auxin’s Hospital At Home works with PCP’s, Mid level providers and Hospitalists . Therefore we maintain the relationship between the Hospitalist, PCP and the patient ensuring a trusted delivery of health care.
Our Hospital at Home program is supported by our mobile unit which is Staffed with trained nurses and equipped with state of the art diagnostic technology as well as all the necessary medications to successfully treat patients as if they were in the ER or Hospital bed.
Our Hospital at Home program is supported by our mobile units. We deliver services on a timely manner that are comprehensive and proven to be effective in avoiding visits to the emergency department and urgent care centers resulting in less hospitalizations.
Our mobile units are equipped with experienced nurses that will conduct the patient encounter. The nurse will connect with one of our virtual Midlevel/Hospitalist or patient’s PCP who will oversee the encounter. The nurse will be able to perform several diagnostic tests using blood work (CBC,BMP,ABG’s, Lactic acid, Troponin, D-Dimer), EKG, cardiac output analysis and Xray. Once a diagnosis has been made the nurse will initiate the treatment. This will include but not limited to pain control, nausea control, cardiac monitoring, vitals monitoring, oximetry , IV fluids, IV medications, IV antibiotics, diuretics, nebulizations, steroids. Our team will be able to treat patient at home for up to 3 hours.
If after 3 hours of intense management the team has not achieve a desired clinical outcome then a decision will be made to refer this patient to a higher level of care for further treatment
Some of the conditions we will be able to diagnose and treat at home:
- Chronic Obstructive pulmonary disease (COPD) exacerbation
- Congestive heart failure (CHF) exacerbation
- Community acquired Pneumonia
- Urinary tract infections
- Asthma exacerbation
- Diabetes
- Localize cellulitis (tissue infections)
- Gastroenteritis/ Dehydration/Nausea/ Vomiting
- Strep Infections/ Influenza (Flu)/COVID-19
Some of the patients treated in our hospital at home program will be required to have daily follow ups to continue diagnostics and treatments until the patient is back to their baseline and can be followed by their PCP.
