Care does not end once a patient leaves the hospital. Post-acute facilities play a key role on safely transitioning patient back home. At Auxin we partner with post-acute facilities to decrease send outs to emergency departments therefore preventing hospitalizations.

Our Virtual visit suit combined with our Remote Telemonitoring program allow healthcare providers identify early patterns that  might trigger an exacerbation of a chronic condition or the appearance of a new disease process.

Furthermore , if needed our mobile care unit team can evaluate and treat patients in the facility , decreasing send outs to emergency departments and hospitalizations.
